MD 34 MCAT 3.89 GPA (1 pub)

HI there. What are my chances of getting into a mid-tier school with these stats.

34Q MCAT (12BS, 10V, 12PS)
GPA (canadian university) 3.89

ECs: Shaddowed a surgeon for 3 days
Medical work team to hospital in Haiti for a week
Lots of music stuff (orchestras, I know 4 instruments pretty well)
Taught a high-school level Biology course for homeschoolers for a year
Research for 4 months with 1 first author publication in a medium impact journal (Plos)
Good possibility for another publication within 6 months

My app is gona be late (still putting my ECs together and haven't touched the essays yet)
I feel I'm lacking because I never volunteered at a local Hospital and stuff....
Any thoughts?
