MD Kindly asking for your help/advice: 3.4 cgpa, 3.3 sgpa, 36mcat

Hi all!

As you can see my stats already in the topic headline, I'm considering whether or not I should apply this application season or do a gap year (either continue working in a research lab, 1 yr MS in physio, or 1 yr MS in public health).

As for my EC's:
1. Intramural sports (6 different teams, various sports)
2. 1-year board position for religious group
3. 1-year board position for ethnic group
4. 1-year social science research
5. 1-year biology research
6. 110 hrs of shadowing (96 spring right before freshman year, 12 during sophomore year)
7. 80 hrs of volunteering at hospital (community service)
8. 60 hrs at Animal clinic (desperate for clinical exposure freshman year)
9. 1 year residence hall government and dorm representative
10. Made quidditch team tryouts but quit for personal reasons
11. 1-year dorm fitness center co-manager
12. various club memberships with no leadership however
13. Dealed with family depression, newly diagnosed diseases

*Eligible for URM status (not ethnically, but economically)

I should note I have also transferred from a state school to a very prestigious top 25 undergrad university if that matters. Grades were slightly higher at state school...

So basically, I would like to know if I have a great shot at attaining an interview from a top tier med school this season or if I should wait for my senior grades, take a gap year and then apply. If you have any suggestions as to where to apply, I would love to hear them!
