MD MD and a few MD/PhDs

I was planning on doing MD-only until this past year when I started thinking seriously about MD/PhD. I really want to apply this summer, but I'm not sure if I'm competitive at all at MD/PhD programs.

I'm thinking about applying MD/PhD at a few schools, basically the ones where you can apply MD/PhD and MD like my state school, UMich, Dartmouth and applying MD at the rest. I'm having a difficult time with my school list. I don't really know what my chances are even for MD at research heavy schools

I'm a Biology major at a state school in the west, graduating in May.

GPA ~3.9
MCAT 35

Research:
-1 year 4 months in a biomedical science lab sophomore year and summer before and after, 2 poster presentations, abstract published in ugrad journal
-4 months and currently working in a different biomedical science lab. Working on my project that is in conjunction w/ a clinical trial, much more interested in research now than during previous experience
-3 semesters clinical research assistant, mainly screening and talking with patients/families about studies (clinical experience?)
-global health study abroad for a few weeks, survey research in third world country, worked on for a few months before and after, national poster presentation and at ugrad

I'm not sure if the last 2 even count as research?

ECs:
-MCAT tutor currently, Bio and Chem tutor for a year before, TA for one semester
-Volunteer at children's hospital for 2+ years
-tutor at junior high schools for students with challenges/parents without college education for one semester
-member of service group freshman year
-volunteer at nursing home (clinical experience?)
-volunteer with refugee family
-plenty of shadowing
-a couple of clubs, hobbies, etc
